Claims Lifecycle
Encounter completed (SOAP notes, CPT/ICD-10 codes)
→ Claim created
→ Submitted to insurance payer
→ Payer adjudicates (approved / denied / partially paid)
→ EOB received
→ Payment posted to Invoice
→ Balance billed to patient (if any)
