What is it?

Single inbox for every prospective patient who has not yet converted into a paying Contact. Captures intake snapshots and ad attribution across four product lines.


Lead vs Contact

LeadCompleted Self Assessment. Has NOT paid. No portal or app access.
ContactHas paid for at least one appointment. Full access.

The conversion from Lead → Contact is triggered by the first payment. The system automatically creates the Contact record in CRM at that moment.


How a Lead is Created

  1. Patient visits one of the assessment flows (Hormone Imbalance, Weight Loss, Peptides, etc.)
  2. Completes the Self Assessment questionnaire
  3. Assessment ends → “Possible Imbalance Detected” screen
  4. Redirected to book an appointment
  5. At this point, if they haven’t paid → they are a Lead

Self Assessment Details

The questionnaire inside the flow:

  1. Symptom selection → each symptom gets a severity rating 1–5
  2. Additional symptoms (open field)
  3. Top 3 goals
  4. Medical exclusions (certain conditions like cancer history exit the flow)
  5. “Calculating Results” → actually a weighted sum of symptom scores
  6. Result screen: “Possible Imbalance Detected” → CTA to book appointment

The “Calculating Results” screen is not actually running complex AI — it’s a summation algorithm to produce a score/percentage display.


Testing as a Lead

When creating test accounts, always use these conventions so test data is filtered from reports:

  • Last name: TL test
  • Account name: Federico TTS
  • Phone: random landline from an online generator (never personal number)
  • State: California or Texas
  • Payment coupon: 100test → 100% discount

Select male when going through Hormone Imbalance — it’s the shortest flow. Female flow asks significantly more questions.


Conversion Event

When the lead pays:

  1. First payment processed (5 insurance)
  2. Zoho automation fires → creates Contact record
  3. Contact inherits data from Lead
  4. Patient gains portal and app access
  5. OTP validation is required for app login
